Fillable PDF ODAFF-1 Oklahoma Official Wood Destroying Insect (WDI) Report

A fillable .pdf file of the Oklahoma Official WDI Report (ODAFF-1 Adopted January 1, 2020) customized with your company information will allow you to easily fill out the report in the field using an iPad, laptop or other hand held multi-touch tablet.  Completed WDI reports can be saved, printed and/or emailed to your customer. There are no monthly or annual fees.  The programming fee is a one-time charge.

This five 8-1/2" x 11" page version has been reviewed and approved by Oklahoma Department of Agriculture Food and Forestry. The first page includes yellow highlighted areas as required by TDA. Form ODAFF-1 Oklahoma WDI Report was released 10/19 and may be used immediately but must be used for WDI inspections in Oklahoma on or after 1/1/20.
