An economical solution for printed HUD-NPMA-99-A Subterranean Termite Protection Builder's Guarantee carbonless forms. This is the new HUD NPMA 99A form released in July 2024. It has an OMB Approval No. that expires 7/31/207. All previous versions of the 99-A forms are no longer valid.

• Form HUD-NPMA-99-A (formerly HUD-NPCA-99-A) is not personalized with any of the pest control company's information as it is not required. The pest control company performing the pre-treatment gives this form to the builder to complete when prevention of subterranean termite infestation is specified by the builder or required by the lender, the architect, FHA or VA. This guarantee is issued by the builder to the buyer.  Form HUD-NPMA-99-A is used in conjunction with HUD-NPMA-99B (when applicable) in accordance with Section 24 CFR 200.926d(b)(3) which requires that sites for HUD insured structures must be free of termite hazards. The licensed pest control company will provide the builder a record of specific treatment information in cases when any method other than use of treated lumber is used for prevention of subterranean termite infestation
