These 4-1/4" x 7" 2-part carbonless Pest Control Service Reports are booked in books of 50 form sets and offer a basic solution for a pest management invoice. NOTE: This stock form is currently only available printed in black ink.  Blue ink is no longer a stock option. • Logo imprint at no additional cost if uploaded in high resolution .eps or .ai vector format (preferable) or 300dpi .jpg or .tiff format (max file size 5 mb);
• Each book contains 50 forms
• Consecutive numbering can be added for no additional cost - just let us know the number you want them to start with;
• Forms ship within 10 business days, unless ordered with a custom printed form or another item that takes longer to produce;
• Allow 3-5 additional days for UPS shipping; and
• No sales tax for orders shipped to addresses outside Texas
